Completing this form will cancel the entire unused portion of your reservation and refund your ticket back to original form of payment. If you didn’t book directly with Hawaiian Airlines, please contact your original ticket issuer for refund requests. Only one refund request is needed for all passengers on the same reservation. Please ensure you provide your confirmation code in the field below or we will not be able to process your request .
